"Blue balls" is a slang term for an aching in the genital area when a guy gets sexually excited but doesn't ejaculate.
 
When a guy is sexually excited, his genital area fills with blood, which helps make the penis erect. This increased blood flow to the genitals also causes the testicles to enlarge. Once a guy ejaculates, the amount of blood in his genitals decreases, and things go back to their normal size quickly. If he doesn't ejaculate, he may have an aching or heavy feeling in the area. But this discomfort is minor, and the sensation goes away quickly on its own.

Some guys will say that they'll be in pain they have an erection but don’t have sex, but "blue balls" is not painful and it won't cause any kind of permanent damage. So don't feel like this is a reason to have sex if you don't want to.
