It's hard to say exactly what could have gone wrong. Most of the time birth control pills and condoms are effective, but not 100 percent of the time. The reality is that other than abstinence, no method of birth control is completely perfect.

Sometimes pregnancy can be the result of something as simple as a tear in a condom so tiny that it can't even been seen. Studies have shown that over the course of a year, 15 out of 100 people who rely solely on condoms for protection will wind up pregnant. It's also possible to become pregnant if unprotected penetration occurs, even if you're on the Pill. For those having sex and wanting to avoid pregnancy, being on birth control is the way to go. But the only truly safe bet for avoiding pregnancy is not to have sex.
